PUPILS PERSONAL DETAILS

  1. The Personal Details Form (obtained from the office) should be completed by all parents and returned to school as soon as possible. This information is very necessary for the care and welfare of your child.
    If there are any changes in the information provided, the school should be contacted to enable us to amend our record.
  2. Medicines - School's Policy
    There are occasions when the doctors consider children; to be fit enough to return to school before their medication is completed. Children needing to take medication during the school day should leave medicines and an accompanying medical form with the Head Teacher. Medicines should be clearly labelled with the child's name and dosage required. It is the parent's responsibility to ensure that the child collects medicines at the end of the day.
    In case of illness or injury at school, the following action will be taken:
    1. The home will be contacted
    2. Nominated adult will be contacted
    3. Parents workplace will be contacted

Any information given will be in confidence. If for any reason the details on the form change, please contact the school immediately so we can adjust our records.

NOTE: an experienced member of staff deals with all incidents that require First Aid.
